Kindergarten Registration 2010-2011

Welcome to the Class of 2023!!

Mayfield Schools is now beginning registration for Kindergarten for the 2010-2011 school year.

If your child will be five years old on or before August 31, 2010, and you are a legal resident of the Mayfield City School District, you must complete a Kindergarten registration packet. You can either
-- download and print the registration packet below; or
-- pick up a registration packet at the elementary school where your child will attend.

Parent(s) and/or guardian(s) will need to schedule an appointment to bring their child to their home school in March.
During the appointment, staff will review the registration materials to make sure all forms are complete and also to complete the vision and hearing screening with your child.

To register, parents and or guardians will need to have
I. Proof of residency (i.e. lease, purchase agreement, utility bills, tax records);
II. Custody papers if applicable;
III. The child’s original birth certificate, and immunization records.
Please bring originals of all items and copies will be made at the time of registration/screening)
