REGISTRATION INFORMATION

TO REGISTER:
A completed registration packet (found at the right of this page) along with required items should be taken to:

Registration hours are: 8:00 a.m. to 3:30 p.m. (no appointment needed)

ATTN: HIGH SCHOOL REGISTRATION
To expedite the scheduling process, please bring to registration a transcript from your previous school. The High School will not be able to make a student schedule until the academic history is received. Thank you.
 

KINDERGARTEN REGISTRATION  2014-15

PLEASE NOTE:  Students entering Mayfield City Schools Kindergarten for the 2014-15 school year must be 5 years of age by August 1, 2014.

Please download a packet at the right of this page.  Bring the completed packet and required items with you to the Board of Education at 1101 SOM Center Rd., Mayfield Hts. 

We will be taking registrations Monday-Friday 8:00am - 3:30pm. 

No appointment is needed


EARLY ENTRANCE TO KINDERGARTEN FOR 2014-15 SCHOOL YEAR

If your child will be turning 5 years of age after the August 1, 2014 cutoff and before December 31, 2014, and you feel they are academically, developmentally, socially and physically ready for Kindergarten you may request Early Entrance Testing for Kindergarten. 

Please send a written request to the Pupil Services Department, Mayfield City Schools, 1101 SOM Center Rd., Mayfield Hts., OH  44124 or by email to rross@mayfieldschools.org  Please include the child's name, birth date, parent's name and address and reason you think your child is ready for kindergarten. Once your request is received we will send out a referral packet.  

Requests for testing need to be received by February 15th

CHANGE OF ADDRESS

  

If you have moved, please fill out the "Change of Address" form located at the right of this page. Bring the completed form and proof of the new residency to the Board of Education at 1101 SOM Center Rd., Mayfield Hts. You can use one form for all students in the household.

 


SCHOOL VISITATION

The Mayfield City School District welcomes visitors to our schools.  In order to provide you with the best visit/tour possible, please complete the "School Visitation Form" and submit it to the Mayfield City School District's Office of Pupil Services at least 48 hours (2 days) prior to the requested visitation date.
