The State of Ohio has made a requirement that all students entering the 12th grade must have two doses of Meningococcal vaccine prior to the start of their Senior year. If your student received their first dose after their 16th birthday, then just one dose of Meningococcal is required.
You may obtain the Meningococcal vaccine from your private healthcare provider or other community providers. Please consider the new guidelines when scheduling your child’s next medical appointment.
Vaccines are also available from:
- The Cuyahoga County Board of Health - 216.201.2041 for an appointment
- Shaker Hts Board of Health - 216.491.1480 for an appointment
- Pharmacies such as CVS, Walgreens etc. Call them first to see if they give vaccines.

Please submit documentation to show that your student received the vaccine after the appointment so that their record in Infinite Campus can be updated to show they have met this requirement. Information can be faxed to the School Clinic at 440-995-6805, dropped off by your student in the Clinic, or emailed to me at firstname.lastname@example.org
