Millridge Elementary is committed to getting your child to the correct destination after school, or signed out properly for early dismissal. This requires direct, timely communication between parents and the school office. These forms will be implemented as a convenience to you instead of a hand written note, and should be filled out each time there is a change in your child’s dismissal routine.
Please complete the required information and send it to school with your child. Please note that both students will need a note when making after school plans(play date).
