Peer Model and Special Needs Screening

Preschool Screening for 2024-2025

We are excited to share the Mayfield Preschool Peer Model Registration and Screening process for the upcoming 2024-2025 school year!!  It is always such an exciting time to meet and learn more about our Mayfield families while we welcome them to the Mayfield City School’s community!  The “Cub House” has the gift of serving our youngest scholars and together with you, lays the foundation for successful schooling experience for years to come.

Should you wish for your child to become a peer model, please review the following steps in the process.  We look forward to working with you during the next few months and getting to know your family!

The questionnaire cannot be completed on a handheld device.  You'll need a computer and internet access to complete it.

2.    Once the completed questionnaire is received:

Parent/Guardian will be contacted with further information regarding Peer Model screening.  A reservation is required for Peer Screening.

ALL prospective peers participate in the peer screening process to be considered as a peer model for the classroom. Following Screening:

Decisions are shared with parents in early March of 2024.  Three possible outcomes can occur:

  1. Accept into Preschool for 2024-25 school year

  2. Waitlist when/if a space is available.

  3. Referral for further evaluation

Children with Special Needs are eligible for enrollment in the program when they reach their third birthday. Evaluation and placement occurs throughout the school year. Children who live outside the Mayfield City School District must have a hearing impairment to be eligible for the program.  Placement is at the discretion of the students home district. Any eligibility is determined by the Rules for the Education of Preschool Children with Disabilities Served by Public Schools (Chapter 3301-31). Our program follows mandated state and local placement procedures. A team of staff professionals reviews the needs of each child to determine the appropriate placement.

Those requesting an evaluation will need to complete the "Referral for Evaluation" form and email that to Darcy Edelman, Preschool Psychologist:  [email protected]
