We are excited to be welcoming our new sixth grade class to Mayfield Middle School and continuing the tradition of sixth grade camp! Our Mayfield Middle School staff is busy planning a great camp experience for your child. We will be going to Camp Fitch the morning of Wednesday, August 28th and returning the afternoon of Friday, August 30th.
The cost for Sixth Grade Camp is $130.00. To register your child, the following items have to be submitted to the Mayfield Middle School Office by Friday, August 16th.

Attendee Release and Parental Authorization Form (must fill out)
Travel Authorization and Medical Treatment of Minors Form (must fill out - 2 pages)
Authorization for Medications to be Taken During Overnight School Trip Form (if needed)
Authorization for Self-Administration of Asthma Inhalers While on Overnight School Trip Form (if needed)
Authorization for Student Possession and Use of an Epinephrine Autoinjector (if needed)

If your child requires the administration of medication during camp, you will need to fill out the Authorization for Medications to be Taken During Overnight School Trip Form. If your child will be carrying an asthma inhaler at camp, you will need to fill out the Authorization for Self-Administration of Asthma Inhalers While on Overnight School Trip Form. If your child will be carrying an Epinephrine Autoinjector, you will need to fill out the Authorization for Student Possession and Use of an Epinephrine Autoinjector.
Forms can be turned into your elementary school’s main office or dropped off to the Mayfield Middle School’s main office any time between 7:30 AM and 3:30 PM on weekdays. Additionally, you can also drop off these items to the Mayfield Middle School main office when you drop off your child for Sixth Grade Orientation in August. Forms can be printed and picked up from MMS at any time.
The deadline to register your child by completing the proper forms and making the payment for Sixth Grade Camp is Friday, August 16th.

Seven athletes, Nick B., Kayin C., Louis D., Joel K., Mikey T., Aidan I. and Ryan C. represented MMS at the Ohio Championship meet Saturday. Congratulations to Nick B. who was 5th in the state in the 100 meter dash and the 4x200 team of Kayin C., Louis D., Joel K. and Nick B. 6th in the state.

entering 7th grade are required to have one dose of Meningococcal vaccine and one dose of Tdap (Tetanus, Diptheria and Pertussis) prior to the start of the school year in August 2019.

If your child has already had these vaccines, please provide an updated vaccine record to the school either by fax 440-449-1413, scan in an email, or sending in a copy with your student to drop off at the Clinic.
