March 2017 Students of the Month

Our Career Day is set for Friday, May 12, from 8:00–10:00 am, here at Mayfield Middle School. Parent Informational Meeting for New York City and Chicago Trips

On Thursday, May 4th at 6:00 pm in the Mayfield Middle School Auditorium there will be a Parent Informational meeting for the New York City and Chicago Trips.  Attendance is optional.

Please select "read more" for the Itinerary for the New York City and Chicago Trips.  The dates of the 8th Grade New York City Trip are May 17th through May 19th.  The dates of the 7th Grade Chicago Trip are May 21st through May 23rd.

Attention Incoming Sixth Grade Students and  Parents

Attention Incoming Sixth Grade Students and Parents

Welcome Incoming Sixth Grade Students and Parents!  We are very excited to be a part of your middle school journey and providing you the opportunity to attend Sixth Grade Camp.  We will be going to Camp Fitch the morning of Wednesday, August 30th and returning the afternoon of Friday, September 1st.  

 

The cost for Sixth Grade Camp is $125.00.  Please make the payment for camp through Infinite Campus.  For instructions on how to do this, click on the document entitled Payment Portal Directions on IC. 

 

To register your child, the following items have to be submitted to their elementary building’s main office by May 19th or to the Middle School Main Office by August 18th:

·        Authorization for Student Possession and Use of an Epinephrine Autoinjector (if needed)

 

If your child requires the administration of medication during camp, you will need to fill out the Authorization for Medications to be Taken During Overnight School Trip Form.  If your child will be carrying an asthma inhaler at camp, you will need to fill out the Authorization for Self-Administration of Asthma Inhalers While on an Overnight School Trip Form. If your child will be carrying an epinephrine autoinjector, you will need to fill out the Authorization for Student Possession and Use of an Epinephrine Autoinjector.

Please make arrangements to drop off the medications during Sixth Grade Orientation on Friday, August 11th from 1PM to 4PM.  Medications must be in the original prescription bottle with the label attached.  Please only put the amount of medication your student will need to take during camp.
