NEW STUDENTS - If you are applying for free or reduced price meal benefits, you MUST provide meals for your children until you receive notification that your child has been approved for such benefits.
Click on the Free & Reduced Meal Application on the right to download a form. Complete the form (Fill out Page 5 carefully and completely or processing will be delayed), and please send it in a sealed envelope to the Food Service Dept at 1101 SOM Center Rd, Mayfield Hts, OH, 44124, ATTN: Robin Smeal.
Once the application is processed you will receive notification by mail only. Please allow 7-10 days for processing. If you were approved last year, your status will carry over this year until an updated application is received or 30 days expires. Thank you.
