Click on the Free & Reduced Meal Application on the right to download a form. Complete the form (Fill out Page 5 carefully and completely or processing will be delayed), and please send it in a sealed envelope to the Food Service Dept at 1101 SOM Center Rd, Mayfield Hts, OH, 44124, ATTN: Robin Smeal.
Once the application is processed you will receive notification by mail only. Please allow 7-10 days for processing. If you were approved last year, your status will carry over this year until an updated application is received or 30 days expires. Thank you.
NEW STUDENTS - If you are applying for free or reduced price meal benefits, you MUST provide meals for your children until you receive notification that your child has been approved for such benefits
