Attention Parents/ Guardians of 6th Graders



All students entering 7th grade are required to have one dose of Meningococcal vaccine and one dose of Tdap (Tetanus, Diptheria and Pertussis) prior to the start of the school year in August 2018.                                                             


Your student can get these vaccines at any of these providers:

1.     Your healthcare provider

2.    Cuyahoga County Board of Health at 216-201-2041 

3.    Shaker Hts. Board of Health at 216-491-1480

4.    Pharmacies such as CVS, Walgreens or Costco

If your child has already had these vaccines, please provide an updated vaccine record to the school either by fax 440-449-1413, scan in an email, or sending in a copy with your student to drop off at the Clinic.

Attention 6th Grade Chorus and 7th Grade Vocal Ensembles


On Tuesday, May 8 the 6thgrade Chorus will present “Rockin’ America,” a musical celebration of pop/rock hits from the 50’s through the early 2000’s! The show will begin at 7:00 p.m. in the lower gym. Admission is free!  Click here 6th Grade Chorus for details.

Sixth Grade Camp 2018 - 2019


We are excited to be welcoming our new sixth grade class to Mayfield Middle School and continuing the tradition of sixth grade camp!  Our Mayfield Middle School staff is busy planning a great camp experience for your child.  We will be going to Camp Fitch the morning of Wednesday, August 29th and returning the afternoon of Friday, August 31st.  

The cost for Sixth Grade Camp is $130.00.  To register your child, the following items have to be submitted to the Mayfield Middle School Office by Friday, August 17th.

If your child requires the administration of medication during camp, you will need to fill out the Authorization for Medications to be Taken During Overnight School Trip Form.  If your child will be carrying an asthma inhaler at camp, you will need to fill out the Authorization for Self-Administration of Asthma Inhalers While on Overnight School Trip Form.  If your child will be carrying an Epinephrine Autoinjector, you will need to fill out the Authorization for Student Possession and Use of an Epinephrine Autoinjector. 

Forms can be turned into your elementary school’s main office or dropped off to the Mayfield Middle School’s main office any time between 7:30 AM and 3:30 PM on weekdays.  Additionally, you can also drop off these items to the Mayfield Middle School main office when you drop off your child for Sixth Grade Orientation or return for the Parent Meeting on August 13th. Forms can be printed and picked up from MMS at any time.  

The deadline to register your child by completing the proper forms and making the payment for Sixth Grade Camp is Friday, August 17th.

2018 Beautiful Lengths


The Mayfield Middle School Beautiful Lengths Project  encourages MMS students, both girls and boys, to grow, cut, and donate their hair in order to make real-hair wigs for girls who have lost their hair due to cancer treatments. 


In order to participate, students (and parents) must complete and sign the attached permission slip.  While there is no due date (we will take permission slips up to the day of the event), to guarantee your child an event t-shirt, slips should be turned in by FRIDAY, APRIL 13.  Click here Beautiful Lengths for a permission slip.
