 Dear Mayfield teachers, staff, students and parents,

As our Focus Forward Mayfield Continuous Improvement Plan moves through its first year, a series of online surveys are set to begin to gather and examine data about how we are improving and where we can get better. The first Focus Forward Mayfield surveys to explore Parent/Caregiver, Student (Gr. 3-12) and Teacher/Staff experiences begin today, November 28th and will be open through December 8th. Please only take the survey once.

The short surveys, conducted by Huron Studer Education, will help us learn more about your experience as a student (Gr. 3-12), teacher, staff or parent/caregiver.  Your responses are anonymous, meaning no one can match your responses with you.  

The survey will take less than 5 minutes to complete. Please take the time to complete the individual survey as it pertains to you: Student (Gr. 3-12), Teacher/Staff or Parent/Caregiver now through December 8th.
