Existing Mayfield Families

This registration page is for EXISTING RESIDENTIAL PARENTS/GUARDIANS with students currently enrolled in Mayfield City Schools that want to ADD a NEW student.

For Current Mayfield Preschool Parents:
If your child is currently enrolled and will be returning to the Mayfield Preschool or entering Kindergarten,
you do NOT need to register again.
They will automatically be enrolled unless you notify us and complete a withdrawal form.
No action is needed until May, when the Annual Update opens.

Only a residential parent/legal guardian may register a new student!
You will need to upload your current government photo ID.
Children must be 5 years old on or before August 20, 2026 to enroll in Kindergarten
Registration Windows:
Registration for all grade levels (Preschool - Grade 12)
for the 26-27 school year is now open. 

Online Registration for existing families is done through the Infinite Campus Parent Portal.
You may add your new student for the 26-27 school year in the Student Section of the 26-27 Annual Update.  
 This ensures all current student information is updated.
Once your Annual Update is completed, if you need to add a new student, please select the 26-27 New Student Registration.

We recommend using a computer or laptop for registration, but you may also use a mobile device if needed.
If you do not have access to the internet, a KIOSK will be provided at the Board of Education office.

Required Documents – For families who already have students enrolled in Mayfield and are adding a new student, the following documents must be scanned and uploaded through the Infinite Campus Parent Portal as part of the application.

  • Photo ID of residential parent/guardian (driver's license or passport)
  • TWO Proofs of Residency (If current address in our system is different, or lease has expired)
    • Mortgage bill or current signed lease agreement with end date and listing occupants
    • Current utility bill [gas, electric, water or sewer], pay stub or bank account
  • Child's Birth Certificate (If not in English, must provide a certified translated copy)
  • Most Current Immunization Records
  • Legal Documents (divorce decree, shared parenting, custody/guardianship papers - if applicable)
  • Special Ed/Gifted Documents (IEP, 504, WEP, WAP - if applicable)
  • Authorization for Release of School Records (if transferring from another school)
  • Medical/Medications (if applicable)
  • Preschool Health Questionnaire (Preschool students only - to be completed by the parent/guardian)
  • Preschool Physical Form (Preschool students only - to be completed by child's physician)

26-27 Annual Update Instructions
Beginning May 1, 2026, please use the 26-27 Annual Update to add a new student for next year and update housing, contact, and medical information. The 26-27 Annual Update is required and must be completed before the first day of school in August.
