NEW MAYFIELD RESIDENT STUDENT PreK - 12

Welcome - we are excited for you to become part of the Mayfield family!

This registration page is for NEW students that reside in the Mayfield City School District whose parent/legal guardian does not have any currently enrolled student in the district.

This includes former students that need to re-enroll.

ONLY A RESIDENTIAL PARENT/LEGAL GUARDIAN CAN REGISTER A NEW STUDENT.

You must show the required proof of residency in the Mayfield City School District.

Children must be 5 years old on or before August 20, 2026 to enroll in Kindergarten.

Registration Windows for NEW to Mayfield:

Registration for all grade levels (Preschool - Grade 12)

for the 26-27 school is now open

We recommend using a computer or laptop for registration, but you may also use a mobile device if needed.

If you do not have access to the internet, a KIOSK will be provided at the Board of Education office.

The following required items are to be scanned and uploaded within the application.

Photo ID of parent/guardian (driver's license or passport)
TWO Proofs of Residency
- Mortgage bill or current signed lease agreement with end date and listing occupants
- Current utility bill [gas, electric, water or sewer], pay stub or bank account
Emergency Contact Phone Numbers at least 1 is required other than a parent or guardian (friend, neighbor, non-household family member)
Child's Birth Certificate (If not in English, must provide a certified translated copy)
Most Current Immunization Records
Legal Documents (divorce decree, shared parenting, custody/guardianship papers - if applicable)
Special Ed/Gifted Documents (IEP, 504, WEP, WAP - if applicable)
Authorization for Release of School Records (if transferring from another school)
Medical/Medications (if applicable)
Preschool Health Questionnaire (Preschool students only - to be completed by parent/guardian)
Preschool Physical Form (Preschool students only - to be completed by child's physician)
