Change of Address

Beginning March 10th, 2025, ALL change of address requests for existing parents of the Mayfield City Schools must be done from the Infinite Campus Parent Portal.
Only the legal residential parent/guardian may request the change of address from the student's primary household.

To change your address:
  • Login to your parent portal account > click MORE > Select Annual Update / Registration / Withdrawal / Address
  • Select ADDRESS CHANGE > click START
  • Within the application select NO to the question 'Is this Address Current' from the Household tab.
  • Required Documents (These documents must be uploaded within this application, delivered in person to the Board of Education, or emailed to [email protected].)
    • Updated Mortgage bill or current signed Lease Agreement with end date and listing occupants
    • Second Proof of NEW residence (utility bill, paystub, bank statement)
    • Photo ID with new address
  • Failure to keep current proof of residency on file with Mayfield City Schools will result in your child being unenrolled from the district.
